Can GARLIC fight cancer?


Since ancient times, garlic has been used for its anti-bacterial and anti-inflammatory curative properties before the arrival of antibiotics.
 
Garlic is a vegetable belonging to the Allium class of bulb-shaped plants, including onions, chives, leeks and scallions. It is commonly used for flavoring in cooking and is unique because of its high sulfur content. Other components found include arginine, oligosaccharides, flavonoids and selenium - all of which are beneficial to health.

The characteristic odor and flavor of garlic comes from sulfur compounds formed from allicin, the major precursor of garlic’s bioactive compounds, which are formed when garlic bulbs are chopped, crushed or damaged. Present in it are 33 different type of sulphur compounds (including Ajoene, allicin), 17 types of amino acids (all essential acids), germanium, selenium, calcium, copper, magnesium, iron, potassium, zinc and vitamin A, B1, C.

Many conclusive studies have shown that garlic extracts are effective in resisting bacteria (staphylococcus, salmonella etc), fungus (candida ablicans, aspergilllus, niger), virus and other pathogens. This is very useful against prevention and treatment of cold and flu too. In fact, a 100mg of garlic extract (Allicin) has the same effect as 10 mg penicillin of anti-bacterial action.

Types of garlic preparations

Garlic fried in oil to add flavor to some main dishes loses much of its medicinal properties that can reduce blood pressure, stroke, heart failure, peripheral vascular disease and renal failure.

Garlic supplements come in different forms:
  • Garlic essential oil is obtained by passing steam through garlic. Commercially available garlic oil capsules generally contain vegetable oil, but only have a small amount of garlic essential oil because of its strong odor.
  • Garlic oil macerate products are made from encapsulated mixtures of whole garlic cloves ground into vegetable oil.
  • Garlic powder is produced by slicing or crushing garlic cloves, then drying and grinding them into powder. This is used as a flavoring agent for condiments and food and is thought to retain the same ingredients as raw garlic.
  • Garlic extract is made from whole or sliced garlic cloves that are soaked in an alcohol solution for varying amounts of time. Also available in powdered form.

Health benefits
+ For cardiovascular disease, garlic is effective in preventing thrombus (a blood clot formed inside an intact blood vessel) and helps to regulate irregular heartbeat. Its Ajoene content has been clinically tested and proven to be able to inhibit the formation of blood clots and regulate fibrinolytic activity, resulting in lowering of triglyceride and LDL cholesterol.

The sulfur compounds in it can bind with plumbum, mercury, cadmium, arsenic and other heavy metals, to be expelled from the body.  As most of the processed foods is laced with harmful substances such as additives, coloring, preservative and flavoring, clearing them is crucial before any major damage occurs to the body.

Can garlic prevent cancer?

Several population studies show an association between high intake of garlic and allium vegetables, and reduced risk of certain cancers, especially cancers of the stomach, colon, esophagus, pancreas, intestines and breast.

However, few clinical trials have been done on humans to examine the potential anticancer effects of garlic to-date. 
 
Allicin, selenium and germanium in garlic have been found to be effective anti-cancerous components. Basically, they block the formation of cancer-causing substances, halt the activation of cancer-causing substances, enhance DNA repair, reduce cell proliferation or induce cell death.

Even for patients who have undergone chemotherapy, garlic helps to inhibit the duplication and biosynthesis reaction of DNA in cancer cell. Since allicin works synergistically with chemo drugs, it may reduce the dosage required, thus minimising their side effects.
 
As all garlic preparations are not the same, it is difficult to determine the exact amount of garlic that may be needed to reduce cancer risk. Also, the active compounds present in garlic may lose their effectiveness with time, handling and processing.

Daily dosage

The World Health Organization’s (WHO) guidelines for general health promotion for adults is a daily dose of 2 to 5 g of fresh garlic (approximately one clove), 0.4 to 1.2 g of dried garlic powder, 2 to 5 mg of garlic oil, 300 to 1,000 mg of garlic extract, or other formulations that are equal to 2 to 5 mg of allicin.

Safety considerations

Although garlic has been used safely in cooking, excessive consumption can cause some side effects, other than strong breath and body odors.

+ Garlic occasionally causes allergies that can range from mild irritation to potentially life-threatening problems. Consuming fresh garlic bulbs, extracts or oil on an empty stomach may occasionally cause heartburn, nausea, vomiting and diarrhea.

+ In some animal and human studies, garlic can lower blood sugar levels and increase insulin. Diabetic patients have to be careful with its usage.

 + Garlic has been shown to interfere with several prescription drugs, especially the HIV medication saquinavir (brand names Invirase® and Fortovase®). In some instances, it can lower the serum levels of saquinavir by almost half.

+ Garlic also acts as a natural blood thinner to be avoided by pregnant women, patients undergoing surgery, and those taking blood thinners, such as warfarin.

+ Garlic bulbs are sometimes contaminated with the bacterium Clostridium botulinum which can grow and produce botulinum toxin in garlic-in-oil products that are not refrigerated and do not contain antibacterial agents.


+ Chemical burns, contact dermatitis, and bronchial asthma can occur when garlic is applied to the skin.

+ Garlic should be avoided by people who are prone to stomach conditions like ulcers, as it can worsen the condition or cause new ones.
 
+  As garlic wipes off our intestinal bacteria like antibiotics, replenishing friendly bacteria is much encouraged.

Gentlemen, if trips to the restroom require sudden dashes or are marked by difficulty urinating, your prostate may be enlarged.


The prostate gland commonly becomes larger in older men in their mid-life termed benign prostatic hyperplasia (BPH). Even though it can cause problems with passing urine, symptoms are often mild. Without treatment, the condition may not deteriorate and improve over time instead. However, if symptoms become troublesome or distressing, a medical review is necessary. What is happening under?

The prostate is part of a man’s sex organs. At birth, a boy’s prostate gland is about the size of a pea. It is about the size of a walnut fully developed by age 25 and surrounds the tube called the urethra, located just below the bladder. Its job is to produce fluid that protects and nourishes sperm cells in semen.

 
The urethra has two functions: to carry urine from the bladder when you urinate and to carry semen during a sexual climax or ejaculation.

Prostatitis refers to the condition where the prostate is inflamed or irritated, causing a burning feeling when urinating and with increased frequency. There may also be a fever or extreme fatigue. Like any inflammation, it is usually a sign that the body is fighting germs or repairing an injury. If prostatitis is caused by bacteria, tiny organisms can be seen through a microscope in a sample of your urine. Often an antibiotic or a medicine that kills bacteria is prescribed.


More worrying is prostate enlargement or BPH where an expanding prostate is caused by changes in hormone balance and cell-growth factors. As it grows, it squeezes the urethra. Since urine travels from the bladder through the urethra, the pressure from the enlarged prostate may affect bladder control. With the bladder weakens and loses the ability to empty itself, urine may stay on for too long to cause urinary tract infection, bladder or kidney damage, bladder stones and on continence.

Medical detection and Diagnosis


Men with such problem normally refuse to seek medical treatment due to the embarrassment and misunderstanding that it may affect their sexual life. Take note that you are not alone. The Urology Care Foundation estimates that 50% of men in their 50s are sharing a similar fate. Seek early treatment before the condition worsens. Three tests commonly conducted are:

1. Prostate Specific Antigen (PSA) - This test checks for protein released by the prostate gland with safe range below 4 ng/mL and higher readings evidenced in benign prostate enlargement or inflammation of the prostate.

2. Digital Rectal Exam (DRE) - The doctor feels the texture of the prostate gland with a finger gently placed in the back passage or rectum. Prostate cancer can cause abnormal nodules to form on the wall of the prostate.

3. Prostate therapy - An ultrasound guided biopsy is performed to obtain tissue samples from the prostate performed under local anesthesia before sending to the laboratory for microscopic examination.

How is BPH treated?

Several treatments are available -
  • Watchful waiting. If your symptoms do not bother you too much, you may choose to live with them rather than take pills daily or have surgery. However, monitor closely and go for regular checkups to ensure your condition does not deteriorate. These steps may help you manage mild symptoms:
        ++ Do pelvic-strengthening exercises.
        ++ Stay active.
        ++ Decrease alcohol and caffeine intake.
        ++ Space out how much you drink rather than drinking a lot at once.
        ++ Urinate when the urge strikes — don’t wait.
        ++ Avoid decongestants and antihistamines.
  • Drugs. The most common type of medications are called alpha-blockers. This class of medications works by relaxing the bladder neck muscles and the muscle fibers in the prostate. The muscle relaxation makes it easier to urinate. You can expect an increase in urine flow and a less frequent need to urinate within a day or two if you take an alpha blocker for BPH.
  • Nonsurgical procedures. For severe condition, where the urethra is totally blocked and the patient cannot urinate or bad urinary tract problem like infection, bladder stones and kidney failure, a more established procedure known as transurethral resection of the prostate (Turp) needs to be performed. It involves inserting thin tubes charged with an electrical current through the urethra to the area where the prostate meets the bladder to scrape out the swollen parts of the prostate around it. With the bladder muscles relaxing, urine can flow out of the bladder easier. A gel may be applied to the urethra to prevent pain or discomfort. You do not need drugs to enable you to sleep. Long-term medication is available but does not always work and has side effects such as decreased libido and enlarged breasts.
  • Surgery is the last option in complicated cases of a very enlarged prostate, difficulty in urinating, having a partial blockage in their urethra that causes repeated urinary tract infections, bladder stones or damage, or those who have kidney damage. Such operation involves removing only the portion of the prostate blocking urine flow. Surgery for BPH may have a temporary effect on sexual function. However, most men recover complete sexual function within a year after surgery. The exact length of time depends on how long you had symptoms before surgery was done and on the type of surgery. After the surgery, some men find that semen is not released from the penis during orgasm. Instead, it flows back into the bladder. In some cases, this condition can be treated with a drug that helps keep the bladder closed. A doctor who specializes in fertility problems may be able to help if backwards ejaculation causes a problem for a couple trying to get pregnant. Erection problems and loss of bladder control can be treated, and chances are good if medical help is sought.
  • Hormonal therapy - Androgens, such as testosterone, are primarily made by the testes and a small fraction is produced by the adrenal glands (located above the kidneys). These hormones are responsible for the development and function of the male sexual organs and distinct male characteristics. Anti-androgens are prescribed for patients undergoing surgical and chemo treatment to disable the hormone from fueling the growth of abnormal cell growth.

PROSTATE CANCER
 
In recent years, prostate cancer is fast becoming the top cancer killer in the world amongst the male population, after colorectal and lung cancer. The incidence of prostate cancer increases substantially with age above 50 years as can be seen from statistics collected:

In many cases, multiple factors may be involved, including adopting sedentary lifestyle, genetics and exposure to environmental toxins such as certain chemicals or radiation. Men with diet high in animal fat and low in soya products are vulnerable. There is also evidence that vitamin D deficiency may increase risk for prostate cancer.




Signs and Symptoms
 
+  Trouble passing urine (frequent urination especially at night, painful urination and difficulty starting and maintaining a steady stream of urine)

+ Blood present in the urine or semen

+ General pain or discomfort in the pelvic area, lower back, hips or thighs, as cancer has spread to bones

+  Weakness or numbness in the legs or feet, even loss of bladder or bowel control from cancer pressing on the spinal cord

+ Erectile dysfunction


Treatment options


Mutations in the DNA lead to the growth of cells in the prostate to start growing uncontrollably and abnormally. The cancer can take the form of a benign growth that does not cause any problems throughout one's lifetime or malignant tumor that turns aggressive and invades other parts of the body.











After diagnosis of prostate cancer, additional tests are required to find out if cancer cells are confined to the prostate or have spread to other regions in the body. This is achieved by bone scan or magnetic resonance imaging (MRI).

The decision on the choice of treatment also depends on the profile of the patient and the aggressiveness of the cancer. As signs and symptoms of prostate cancer tend to show up late, it may be already in the advanced stage upon first detection.

+ In patients where prostate cancer is involving other organs and where medical care is not possible, they are prescribed hormonal therapy to control symptoms and improve quality of life.

+  As the male sex hormone testosterone fuels the growth of prostate cancer cells, removing it will result in tumor cells not being able to survive. For those with severe cancer profile and in good health, surgical procedure may necessitate the removal of prostate, both testes, surrounding tissue and seminal vesicles. In some instances, the pelvic lymph nodes around the prostate gland may also be removed. Usually a 15cm incision below the navel in the midline of the abdomen is performed.

+ Radiation therapy entails the use of high energy X-rays to kill cancer cells. Hormonal therapy is usually applied at the same time.

+ Chemotherapy therapy is usually given to patients with advanced cancer not responsive to hormonal treatment. This modality involves drugs used to stop the growth of cancer cells, either by killing or stopping them from dividing.

+ In more advanced stage, highly potent drugs have to be prescribed when conventional treatment like chemotherapy and hormonal therapy are no longer effective.

+  Vaccine therapy and bone-directed therapies may also be necessary for some patients.

For all the above treatment options be prepared to endure discomfort and pain from complications.

The Natural Way to Solve Prostate Problem

Recent study from a German medical team shows that men who subject themselves to repeated biopsies to diagnose prostate cancer could suffer from temporary erectile dysfunction (side effect decreases with time) and in some cases, lingering problems with urination, such as straining to pass urine and frequent night-time trips to the toilet.

Taking more samples during the biopsy should increase the chances of finding any tumor. However, all those needle sticks may come with a cost due to the damage to the "neurovascular bundle" a maze of nerves and blood vessels close to the prostate.

There are a number of well-controlled clinical studies that confirm the safe and effective use of saw palmetto for the treatment of BPH. Saw palmetto extracts can reduce the activity of 5-alpha-reductase by about 40%, and hence reduce the uptake of dihydrotestosterone by the prostate gland. The active substances in saw palmetto berries, such as the phytosterols, provide beneficial effects such as increased urinary flow, increased ease in commencing urination, reduced post-voiding residual volume, decreased frequency of urination, and a decreased urge to void during the night. While saw palmetto effectively diminishes the symptoms of BPH, it appears to act without reducing the size of the prostate.

Experiments have shown that men with an enlarged prostate who received an extract of saw palmetto for 30 days experienced 45% less nocturnal trips to the bathroom, a 50% increase in urinary flow rates, diminished residual urinary volume, and less pain during urination. After three months 90% of the patients reported much lesser problems with no side effects.

Our comments

Making dietary changes and regular exercises may slow the progress of prostate disorders. Men should increase the intake of vitamins and minerals from whole food, such as flaxseed, tomato and soybean. Low fat and high fiber diet are no longer important to just women, but also men. Beside, health authorities should educate public on the importance of healthy prostate and encourage them to be brave when faced with such 'embarrassing' issue.

    GASTRIC Cancer

    Several different types of cancer can occur in the stomach. The most common type is called adenocarcinoma, which starts from one of the common cell types found in the lining of the stomach.
     
    Causes and risk factors

    Scientists have found several risk factors that make a person more likely to get stomach cancer. Some of these can be controlled (eg smoking)  but others cannot (eg. age, family history)

    Gender - Stomach cancer is more common in men than in women - up to three males are affected for every female (estrogen may protect women against the development of this cancer form.)

    Age - There is a sharp increase in stomach cancer rates in people over the age of 50. Most people diagnosed with stomach cancer are between their late 60s and 80s.

    Ethnicity - Globally, stomach cancer is more common in Japan, China, Southern and Eastern Europe, and South and Central America. This disease is less common in Northern and Western Africa, South Central Asia and North America.


    Helicobacter pylori infection - People with stomach cancer have a higher rate of H pylori infection than people without this cancer. Even so, most people who carry this germ in their stomach never develop cancer.

    Diet - An increased risk of stomach cancer is seen in people with diets that have large amounts of smoked foods, salted fish and meat, and pickled vegetables. Nitrates and nitrites are substances commonly found in cured meats. They can be converted by certain bacteria, such as H pylori, into compounds that have been shown to cause stomach cancer in lab animals. On the other hand, eating lots of fresh fruits and vegetables appears to lower the risk of stomach cancer.

    Smoking -Smoking increases stomach cancer risk, particularly for cancers of the upper portion of the stomach near the esophagus. The rate of stomach cancer is about doubled in smokers.

    Obesity - Being overweight or obese is a possible cause of cancers of the upper part of the stomach nearest the esophagus. In recent times, the number is rising for the deadly gastric cancer associated with high-fat diets amongst the young population who are fond of junk foods. It is so aggressive that only 2 out of 10 can survive after 5 years as compared to 6 in 10 for the common gastric cancer. Smoking and drinking complicate the problem. Obese people can develop heartburn that worsens the condition.

    Existing growth - Have a polyp larger than 2 cm in your stomach or inflammation and swelling of the stomach for a long time.


    Previous stomach surgery - Stomach cancers are more likely to develop in people who have had part of their stomach removed to treat non-cancerous diseases such as ulcers. This might be because the stomach makes less acid, which allows more nitrite-producing bacteria to be present. Reflux (backup) of bile from the small intestine into the stomach after surgery might also add to the increased risk. These cancers typically develop many years after the surgery.

    Pernicious anemia - Certain cells in the stomach lining normally make a substance called intrinsic factor (IF) that we need to absorb vitamin B12 from foods. People without enough IF may end up with a vitamin B12 deficiency, which affects the body’s ability to make new red blood cells and can cause other problems as well. Along with anemia (too few red blood cells), people with this disease have an increased risk of stomach cancer.

    Blood type A - It has been consistently observed that blood type A is associated with an increased risk for stomach cancer and poorer survival. On the contrary, Blood type O appears to exert a protective effect by preventing the growth and spread of the tumor and being associated with longer survival times.

    Inheritance - Have a family history of gastric cancer (in 10% of cases reported)

    Symptoms



    Stage 1 (Early) – Limited to the inner lining of the stomach
    • Indigestion or a burning sensation (heartburn)
    • Loss of appetite, especially for meat
    • Abdominal discomfort or irritation
    Stage 2 (Middle) - Penetration to the second and/or third layers of the stomach and nearby lymph nodes
    • Weakness and fatigue
    • Bloating of the stomach, even after small meals
    Stage 3 (Late) – Penetration to the third and/or fourth layer and more distant lymph nodes or has metastasized to other organs
    • Abdominal pain in the upper abdomen
    • Nausea and occasional vomiting
    • Diarrhea or constipation
    • Weight loss
    • Bleeding (vomiting blood or having blood in the stool) which is blackish, leading to anemia.
    • Tumor in the extension of the gastric tumor in to the esophagus.
    Note that these can be symptoms of other problems such as a stomach virus, gastric ulcer or tropical sprue.

    Signs and tests

    + Not everyone has warning signs such as stomach pain and loss of appetite.

    + Symptoms may not occur in the early stages of the disease or patients may self-treat symptoms that gastric cancer has in common with other, less serious gastrointestinal disorders (bloating, gas, heartburn and a sense of fullness). This causes a delay in diagnosis and treatment.

    The following tests can help diagnose gastric cancer:
    • Complete blood count (CBC) to check for anemia
    • Endoscopy to examine the stomach tissue
    • Stool test to check for blood in the stools
    Gastric cancer can spread to the liver, the pancreas and other organs near the stomach as well as to the lungs, the doctor may order a CT scan, a PET scan, an endoscopic ultrasound exam, or other tests to check these areas.

    Treatment

    + At an advanced stage, is recommended to perform chemotherapy and surgery to remove the entire stomach as well as the surrounding lymph nodes, with the basic purpose of removing all cancer and a margin of normal tissue. Depending on the extent of invasion and the location of the tumor, surgery may also include removal of part of the intestine or pancreas. For aggressive cancer, the relapse rate is higher than for common stomach cancer.

    + For patients who cannot have surgery, chemotherapy or radiation therapy can improve symptoms and may prolong survival, but will likely not cure the cancer. Both treatments are better applied after surgery to improve the chance of a recovery.
     
    Chance of recovery
    Tumors in the lower stomach are cured more often than those in the higher stomach around the gastro-esophageal junction. How far the tumor invades the stomach wall and whether lymph nodes are involved when the patient is diagnosed affect the chances of a cure.

    When the tumor has spread outside the stomach, a cure is not possible and the goal of treatment is to improve symptoms.
     
    Prevention

    Mass screening programs have been successful at detecting disease in the early stages in Japan, Korea and Taiwan where raw foods are commonly consumed.

    The following may help reduce your risk of gastric cancer:
    • Avoid smoking

    Colorectal Cancer (IV)

    Bowel movement is a private activity taboo to discuss for majority of the people.  As a result, when they get anal bleeding, it is merely passed off silently without informing their family members or consult a doctor.  Instead, they may treat the symptom with ineffective 'pile cures'.

    Colon structure
     
    The large intestine is made up of the colon (upper) and the rectum (lower). Being the last stage of digestion, it merely absorbs water from the indigestible residue of food to be recirculated into the body and extract important vitamins such as vitamins K and B12, riboflavin and thiamine. The waste is stored in the rectum before sending out to the anus as faeces.
     
    How cancer develops

    As colorectal cancer currently ranks third most common cancer worldwide and fourth leading cause of cancer, leading a healthy lifestyle is of paramount importance and taking all necessary prevention is better than cure, especially those over their prime age.

     
    Cancer of the colon and rectum (colorectal cancer) starts when the process of the normal replacement of lining cells goes wrong. Mistakes in mucosal cell division occur resulting in cells dividing independently of the normal checks and balances that control growth. As these abnormal cells mutate, they can lead to growths within the colon known as polyps.
     
    Polyps are usually precancerous tumors that grow slowly over the course of years and remain localised for a long time. As polyps grow, additional genetic mutations further destabilize the cells and may invade other layers of the large intestine turning it cancerous. This is a gradual process and takes more than 8 to 10 years to be fully blown. Hence, early detection should see 3 out of 4 patients with colorectal cancer saved.

    Two possibilities can happen.
     
    First, the cancer can grow locally and extend through the wall of the intestine and invade adjacent structures, creating a bigger mass harder to remove. This may bring symptoms such as pain or fullness, or cause blockages of the colon or nearby structures. In advanced stage, the 5 year survival rate is under 50%.
     
    Second, as the cancer grows and metastasise, shedding thousands of cells a day into the blood and lymphatic system that can cause cancers to form in distant locations. In fact, colorectal cancer usually spreads first to local lymph nodes before traveling to common destinations like the liver, the abdominal cavity and the lung.

    Symptoms
     
    When colorectal cancer first develops, there may not be symptoms at all. As the cancer grows, watch for common signs and symptoms like:
    • a change in the frequency of bowel movements
    • diarrhea, constipation or feeling that the bowel does not empty completely
    • either bright red or very dark blood in the stool (rectal bleeding)
    • stools that are narrower than usual
    • general abdominal discomfort such as frequent gas pains, bloating, fullness and/or cramps
    • significant weight loss
    • constant tiredness
    • vomiting
    • unexplained anemia
    The patient tends to look pale, easily tired and often breathless after moderate exertion.

    Risk factors

    Males are generally meat eaters, rendering them prone to colon cancer. Factors that increase a person’s risk of colorectal cancer include:

    (1) High fat intake - It is believed that the digestion of fat that occurs in the small intestine and the colon leads to the formation of cancer-causing chemicals (carcinogens).

    (2) Family history - Approximately 20% of colon cancers are hereditary. Chromosomal defects inherited at birth develop into polyps and/or cancer as early as infancy.

    (3) Accumulation of toxins likely due to other diseases eg. diabetes. 

    Prevention

    Study showed majority (80%) of colon cancer patients do not have family history. Hence, it has to do with lifestyle choices which are controllable to a large extent.

    + Two contributing factors are alcohol usage and smoking.

    -  Moderate and heavy drinkers have a 20% and 50% higher chance of getting this cancer as compared to non-drinkers.

    -  Smokers have 18% higher risk of developing colorectal cancer and 25% higher risk of dying from cancer as compared with non-smokers.

    + People can change their eating habits by reducing fat intake and increasing fiber in their diet.
     
    -  Major sources of fat are red meat, eggs, dairy products, salad dressings and oils used in cooking.
     
    -  Fiber is the insoluble, nondigestible part of plant material present in fruits, vegetables, whole-grain breads and cereals. With the bulk provided, fiber leads to the more rapid transit of fecal material through the intestine, thus allowing less time for a potential carcinogen to react with the intestinal lining.
     
    + While consumers are told to cook meat thoroughly to remove pathogens that are eliminated by intense heat, there are dangers from overcooking.

    -  Dripping fat from frying, broiling and grilling protein foods like beef, pork, poultry and even fish can produce cancer-causing compounds linked to stomach, colon, bladder and several other cancers.

    -  Avoid processed foods like bacon, luncheon meat, ham and hot dogs that are pre-treated with nitrites to kill food-borne germs and maintain a natural color. Carcinogens appear when nitrites become nitrosamines.

    + Drinking chlorinated water has been linked to a great incidence of colon cancer. On the other hand, some researchers believe coffee has positive effects on reducing colon cancer risk.

    + Studies have shown the risk of developing colorectal cancer is 1.5 fold higher for obese people over people of normal weight. To combat this cancer, regular exercise can possibly reduce the risk by 20%.

    + As colorectal cancer displays no early symptoms, screening can help to detect abnormal growths. People above 50 years old are advised to take an annual faecal occult blood test (FOBT) and colonoscopy every 10 years.

    Treatment

    As mentioned, the earlier the cancer is detected, the higher the chance of cure.

    -  For Stages 1 to 3, surgery is recommended. It involves truncating a portion of the large intestine and reconnecting to the healthy segment of the organ. The survival rate for Stage 1 to 2B can be as high as 80-90%. Follow-up chemotherapy is usually arranged to clear residual cancer cells that may not have been surgically removed with the tumor.

    -  For tumor occurring in the rectum around the anus, the patient needs a stoma instead. This is a surgically created opening which connects the large intestine to a pouch on the exterior body where faecal matter is collected and disposed off.

    -  At the final stage 4, survival rate is as low as 5% only. As this is a complicated process, specialists involved may include surgeons, oncologists and radiation oncologists. Chemotherapy is applied to control the spread of wildfire, reduce the tumor size and relieve symptoms but no promise to arrest the deteriorating condition.

    Recommendations

    Besides avoiding the risk factors mentioned above, do observe methods of food preparation:

    - Cooking should be done simply by steaming or little oil used.

    - Microwave ovens should be avoided due to the potential low-level radiation leakage damaging foods leading to stomach and intestinal cancerous growths.

    - The best method of eating is raw. It has been found that cancer patients who used fresh raw cabbage and carrot juice reported excellent results.

    - Consume as little barbecued foods as possible as high temperature cooking results in certain chemicals with carcinogenic effect formed.

    Rectal BLEEDING (III)

     

    The rectum is the last portion of the large bowel that ends just before the anus. Bleeding from this area can be a mild or life-threatening matter. Thus, the presence of bleeding must be carefully checked to avoid unpleasant consequences.

    Colour of blood stool

    Blood in the stool can be bright red, maroon or black. Causes of blood in stool range from harmless, annoying conditions of the gastrointestinal tract such as hemorrhoids and polyps to serious conditions such as cancer.

    The color of the blood during rectal bleeding often depends on the location of the bleeding in the gastrointestinal tract.

    + Generally, the closer the bleeding site is to the anus, the blood tends to be a brighter red. Thus, bleeding from the anus, rectum and the sigmoid colon are usually bright red;

    + Bleeding from the transverse colon and the right colon (transverse and right colon are several feet away from the anus) tend to be dark red or maroon colored;

    + Worst is bleeding that is black, sticky and foul smelling known as melena. It usually signifies bleeding is from the upper gastrointestinal tract (like bleeding from ulcers in the stomach or the duodenum or from the small intestine) as blood usually remains up there longer, allowing more time for bacteria to break it down into chemicals, blackish in color.

    In rare instances, massive bleeding from the right colon, small intestine, or ulcers of the stomach or duodenal can cause rapid transit of the blood through the gastrointestinal tract, resulting in bright red rectal bleeding. Possibility is someone who has swallowed a foreign body that causes injury to the stomach lining, bleeding stomach ulcers, or cuts/ruptures of vessels in the lining of the esophagus which induces continual or forceful vomiting.

    For anemic patients with lost of much iron, bleeding from the gastrointestinal tract can be so slow as to cause either rectal bleeding or melena. In these patients bleeding is occult not visible to the naked eyes and can only be verified by testing the stool for blood in the laboratory. The causes and symptoms are quite similiar as rectal bleeding.
    Causes of Rectal Bleeding
    + Rectal Fissure

    Mild rectal bleeding of bright red blood occurs due to a tear in the lining of the rectum caused by the passage of hard stools. The exposed nerves and vessels cause moderate-to-severe pain especially during bowel movements.

    + Colon polyps/tumor/cancer

    For polyps and tumors, rectal bleeding is less frequent. If it does occur, usually slow, chronic and minimal.
    • Polyps: Lumps of tissue or polyps bulge out from the lining of the colon. Bleeding occurs when large polyps develop. Usually harmless, some types can be pre-cancerous.
    • Tumors: Both benign and malignant forms are frequently found in the colon and rectum. Most sufferers are above 50 years but can also affect younger people too.
    • Colorectal cancer: A growth left untreated on the lining can grow into the muscle layers underneath, and then through the bowel wall.
    + Hemorrhoids






    Hemorrhoids are swollen rectal veins in the anal (external) and rectal (internal) area that can cause burning, painful discomfort and bleeding as a result of hard stools and straining with bowel movements. Treatment focuses on relieving these symptoms with the use of stool bulking agents and softeners.

    + Diverticulitis (Diverticulosis)

    Diverticula are protusions in the weak points of the intestinal wall of a constipated patient where high pressure is constantly applied to force small portions of hard, dry stool through the intestine due to weak bowel muscles, insufficient fibre and water intake. There is no arising problem unless tiny cuts in the pouches become infected or inflamed, causing fever, chills, nausea and pain, which may require surgical removal.

    + Bacterial infection

    Bacterial dysentery is commonly the source of infectious, bloody diarrhea caused by bacteria like Salmonella, Shigella, Escherichia coli, and Clostridium difficile. The symptoms include abdominal pain, fever and bloody diarrhea, commonly treated with antibiotics.

    + Angiodysplasia

    This is a vascular problem that involves enlarged veins and capillaries in the wall of the right colon found mainly in elderly people. These areas become fragile and the bleeding is usually painless, slow, chronic and not obvious until massive bleeding occurs. Sufferers complain of weakness, fatigue and shortness of breath.

    + Peptic & Gastric Ulcer

    A peptic ulcer usually arises in the duodenum or first part of the small intestine, just after the stomach. This area is rather acidic and thus gives extreme pain. As many as 70-90% of ulcers are associated with Helicobacter pylori, a spiral-shaped bacterium that lives in the acidic environment of the stomach. Ulcers can also be caused or worsened by drugs such as aspirin, Plavix, ibuprofen and other NSAIDs (non-steroid anti-inflammatory drugs).

    Though only 4% of duodenal ulcers are malignant, medical attention is needed if there is vomiting of blood due to bleeding directly from a gastric ulcer, or from damage to the esophagus.

     

    Critical conditions


    Immediate medical attention is required when rectal bleeding is present with one or more of the following symptoms:
    • Fever
    • Stomach pain or swelling
    • Nausea or vomiting
    • Bleeding continues or worsens
    • Recent weight loss
    • Altered bowel habits
    • Severe or prolonged diarrhea
    • Pencil-sized stools, involuntary seepage of stools, or difficulty in passing motion
    Emergency conditions

    If any of these signs and symptoms are present -
    • Black or maroon stools
    • Large volume blood loss
    • Rectal pain
    • Dizziness, weakness, or fainting spells
    • Rapid or irregular heartbeat
    • Difficulty breathing
    Other comments


    The majority of people with significant rectal bleeding are elderly who have many other medical problems as well. As a result, they tend to suffer increased rates of illness and death.


    Fortunately, In recent years, death from rectal bleeding has significantly decreased largely due to more efficient emergency departments, recent advances in procedures and evolving surgical management.

    Though about 80% of acute rectal bleeding episodes will resolve without treatment, the chance of recurrence is 25%. This underscores the need for making a definitive diagnosis and in discovering the source of the bleeding so that the corrective actions can be taken at an early stage.

    What should be a cancer patient's DIET?

     

    A cancer patient is usually found to be lacking in nutrients without realising that good nutrition is crucial to keep the body going. In designing an appropriate diet for both the illness and its treatments, place consideration on food tolerance and usage of nutrients. Studies showed that with right mix of foods, many cancer patients can achieve desirable results to suffer lesser or even recover gradually.

    At different stage and type of cancer, the requirement of nutrients varies for individuals. However, it is found that in all cases, eating well during treatment is important to -

    + Feel better
    + Keep up the strength and energy level
    + Maintain the weight and body’s reserve of essential nutrients
    + Better tolerate treatment-related side effects to lower the risk of infection
    + Heal and recover faster

    Diet to consider

    (1)  If a cancer patient is at the terminal stage with days numbered, eating wisely is no longer relevant.

    (2)  As for patients in the early stages, it is advisable to reduce the intake of unhealthy foods such as junk foods, processed refined foods, saturated fats, salt, sugar or white flour. Also leave out acidic foods such as animal meat, dairy products, alcohol and caffeine till much improvement is seen.

    The top cancer-causing foods to avoid-

    Hot dogs. As they are high in nitrates, the Cancer Prevention Coalition advises that children eat no more than 12 hot dogs a month. If you cannot live without hot dogs, buy those made without sodium nitrate. 

    *  Equally high in the same sodium nitrates found in bacon and other processed meats raise the risk of heart disease. The saturated fat in bacon also contributes to cancer.
     
    *  Both doughnuts and french fries are made with hydrogenated oils and then fried at high temperatures produces cancer-causing acrylamides.   

    Chips, crackers and cookies are usually made with white flour and sugar. Even the ones whose labels claim to be free of trans-fats generally contain small amounts of trans-fats.

    A healthy diet incorporates a variety of foods to avail the body a wide array of nutrients much needed to combat cancer. It should comprise protein, carbohydrates, fat, water, vitamins and minerals.

    Proteins

    Body cells are made of amino acids, derived from protein. Thus, protein is essential for growth, to repair tissue and keep our immune system healthy. Without sufficient protein, the muscles are broken down to provide the fuel energy. This delays the recovery process and increases the vulnerability to infection. As such, cancer patients need more protein than normal, especially after surgery, chemotherapy or radiation therapy.

    Good protein can be derived from fish, poultry, lean red meat, eggs, low-fat dairy products, nuts, dried beans, peas lentils and soy foods. Incidentally, many types of nuts, beans and meat contain an important anti-cancer mineral -selenium.

    Carbohydrates

    Carbohydrates are energy giver needed for physical activity and proper bodily functioning.

    Good sources of carbohydrates are fruits, vegetables and whole grains which incidentally supply plenty of vitamins, minerals, fibre and phytonutrients required at the cellular level.

    Fats

    Do not ignore fats as it plays an important role in nutrition too. Fats and oils are made of fatty acids that provide energy to the body. In fact, the body breaks down fats and uses it to store energy, insulate body tissues and transport some types of vitamins through the blood.

    Not all fats are equal. Good ones such as monounsaturated and polyunsaturated fats have positive effects on the heart and cholesterol level whilst saturated fats and trans-fats are considered bad. Monounsaturated fats are found mainly in vegetable oils including olive, canola and peanut oils. Polyunsaturated fats are found mainly in seafood and vegetable oils like safflower, sunflower, corn and flaxseed.

    (However, note that high consumption of polyunsaturated fats causes low cholesterol levels, leading to an increased risk of dying from cancer)

    Water

    Water is necessary for normal body functioning, particularly the heart and digestive system. It also provides moisture to our lung tissue to facilitate oxygen intake and carbon dioxide excretion. Thus it plays a crucial role in helping the body to purge the excess metabolic waste so that our cells can be reactivated.

    Pure fruit juices are preferred over bottled fruit juices that are loaded with fructose or sucrose to improve the taste, even though they may contain many nutrients. A sudden rush of sugar triggers the insulin response, making one tired as the extra calories are converted into fats if not expended.

    Vitamins and Minerals

    Vitamins and minerals enable our bodies to work properly. The critical biochemical functions performed by them are found to take place on a molecular versus a cellular level. Vitamins are organic substances made by plants or animals whereas minerals are inorganic elements extract from the soil and water, and are absorbed by plants or eaten by animals.

    Look for those brightly colored fruits and vegetables loaded with phytochemicals. They may act as antioxidants (nutrient protectors) or prevent carcinogens (cancer causing agents) from forming.

    The powerful antioxidants include Vitamin A, C, E, beta-carotene, lycopene and lutein. Minerals such as selenium and germanium are clinically tested to handle cancer well. Onions, pearl barley, ginger and garlic are known to contain such anti-cancer properties.
     
    Eat plenty of cruciferous vegetables like broccoli, cabbage, cauliflower and spinach. Also add on yellow and deep-orange vegetables like tomatoes, carrots, pumpkin, squash and yam. As for fruits, apples, berries, Brazil nuts, cherries, grapes, oranges and plum are good choices.

    Lung Cancer (with testimony)

      

    According to the Singapore Cancer Registry, lung cancer is the second most frequent cancer in men (after colon cancer) and the third most common type of cancer in women (after breast and colon cancers). We tend to associate lung cancer with smokers as cigarette smoking increases the chance of contracting lung cancer by 10 to 15 times higher than a non-smoker.

    Types

    Lung cancer can be divided into 2 main types:  small cell and non-small cell.  Virtually all smokers and ex-smokers have the small cells that are considered aggressive in nature and tend to metastasize and spread rapidly to other parts of the body. With much conscious efforts put in to discourage smoking, there is a noticeable drop in such cancer cases.

    On the contrary, is alarming to hear that the number of lung cancer patients who are non-smokers is rising.  Many women are caught unexpectedly by this disease (suspect to be smoke inhaled from cooking) since there are no early symptoms, making the all-round survival rate relatively low.

    Causes

    Associated risk factors include:

    -  marijuana use,
    -  exposure to secondary smoke,
    -  exposure to pollution, asbestos, nickel, chromates or radioactive materials,
    -  alcohol consumption,
    -  chronic bronchitis,
    -  history of tuberculosis,
    -  lung scarring from pneumonia and
    -  deficiency in Vitamin A.

    Symptoms

    The symptoms of both types are quite similar.

    -  persistent cough (can be with blood-stained sputum), chest pain, shortness of breath, wheezing, hoarseness,
    -  unintended weight loss,
    -  loss of appetite,
    -  recurring pneumonia,
    -  fever for unknown reason.

    If any of the symptoms do not go away after a couple of days, a biopsy may need to be performed.  If tested positive, treatment typically involved surgery, radiotherapy and chemotherapy will be recommended according to the severity and stage of development.

    Diagnosis

    +  Sad to say that only 20% of non-small lung cancer is detected at early stages of I and II where surgery is still possible to remove the tumor along with part of the normal lung.  In this group, only half are cured and the half suffer relapse and eventually die.
    +  About 1/3 of lung cancer patients are diagnosed with stage III and are treated with radiotherapy/chemotherapy to increase the chance of long-term survival.
    +  About half of the patients discovered too late and at stage IV, chemotherapy has to be applied to control the spread of the disease, to prolong survival as well as manage and prevent symptoms and/or complications.

    As this is an incurable disease, the chance of survival is at best only 20% for up to 2 years.

    What to do?

    A diet high in fruits and vegetables may help to improve the condition. Tomatoes, shiitake mushrooms and  soybeans contain potent antioxidants that may have inhibiting effect on the growth of lung cancer cells.

    Given the high fatality rate due to late diagnosis in most cases and poor results with current available treatments, it pays to be more vigilant about the common symptoms and seek medical advice as early as possible.
